Subject Access Request

If you would like a full copy of your medical record, you can request a subject access request (SAR). This is free and we have 30 days to process your request from acknowledgement of receipt. A third party such as a solicitor can request a Subject Access Request on your behalf but must have written and signed consent from yourself. A fee may occur to your request if you request excessive amounts of subject access requests within a short window of time. We are moving towards a paperless system and all our Subject Access Requests are sent via secure email unless you stipulate otherwise.
